Global Healthcare RCM Provider Reduces Cloud Costs by 27%, Saving $491,000 Annually

About the client

Our client is a global healthcare Revenue Cycle Management (RCM) and medical billing provider, headquartered in the United States with operations across North America, Europe, India, Africa and the Middle East. The company serves physician practices, anesthesiology groups, labs and pathology providers, behavioral health organizations, EMS agencies, hospitals, and surgery centers. Its business applications and virtual desktop estate run across Azure, AWS and Oracle Cloud Infrastructure (OCI), supporting billing and RCM teams around the clock.

Business challenges

As the client expanded across regions and added clouds, its estate grew to 100+ subscriptions and accounts and 2,272 resources spread across US-East, US-West and Central India. Each cloud was managed in isolation, with its own tools, owners, and reporting. Cloud spend kept rising and limited visibility affected budgeting, accountability, forecasting, or optimization speed, but no one could say with confidence which services, teams or applications were driving it.

The client partnered with Aspire Systems to address the following challenges:

  • No unified view of multi-cloud spend Cost data sat in separate consoles for Azure, AWS and OCI. Without a single inventory or cost lineage, finance and engineering teams could not trace spend back to services or business owners, and optimization decisions were made on partial information.
  • Idle and oversized resources Unused VMs, unattached disks, stale snapshots, orphaned load balancers and underused Kubernetes capacity accumulated across subscriptions. Many workloads, including Azure Virtual Desktop (AVD) session hosts, ran on VM sizes well beyond what their usage required.
  • An inefficient virtual desktop estate The AVD host pools used depth-first load balancing, which filled hosts one at a time and kept far more session hosts running than the user load needed. Because these desktops support live billing operations, any change created a real risk of disrupting users.
  • Weak cost governance Inconsistent tagging made chargeback impossible; storage sat in hot tiers regardless of access patterns, and there was no roadmap for Reserved Instances or Savings Plans. Cleanup efforts were one-off exercises with no operating model to sustain them.

Solution approach

Aspire Systems delivered an end-to-end FinOps engagement aligned to the FinOps Foundation framework, running the Inform, Optimize and Operate phases across AWS, Azure and OCI as a single integrated workstream. Rather than treating each cloud as a separate cleanup project, our FinOps specialists built one cost model, one tracker, and one governance process for the entire estate.

1. Inform: building a single source of cost truth

Within three months, the client had service-level cost trending across all three clouds for the first time. Our team began with cross-cloud inventory and cost-lineage discovery, mapping every resource to its subscription, service and owner. Using Azure Cost Management and Advisor, AWS Cost Explorer and Trusted Advisor, OCI Cost Analysis and Power BI, we consolidated findings into a unified Cost Optimization Tracker covering 2,272 line items.

2. Optimize: removing waste and re-architecting for efficiency

With full visibility in place, we prioritized savings by impact and risk:

  • Idle cleanup of unused VMs, disks, snapshots, load balancers and Kubernetes resources
  • Right-sizing AVD session hosts from D-series to burstable B-series VMs
  • Re-architecting AVD host pools from depth-first to breadth-first load balancing, retiring 90+ session hosts
  • Storage tiering from Hot to Cool and Archive based on access patterns
  • A Reserved Instance and Savings Plan commitment roadmap for steady-state workloads

3. Operate: making savings sustainable

Because the estate supports live billing operations, every change went through an approval workflow with business and subject matter expert sign-off and was executed in agreed maintenance windows. This delivered zero downtime throughout the engagement. We also introduced tagging and chargeback governance and handed the client a continuous-optimization tracker so its teams can keep finding and acting on savings.

Best practices embedded

  • Approval-gated change control
  • Tagging discipline across all clouds
  • Cross-cloud cost normalization
  • A VDI migration roadmap to OCI
Value adds

Value adds

  • Delivered multi-cloud FinOps across AWS, Azure and OCI as a single workstream, with deep AVD/VDI expertise.
  • Replaced fragmented consoles with a unified FinOps tracker across all clouds.
  • Shifted the client from tactical cleanup to a strategic cloud cost roadmap.
  • Built a sustainable FinOps operating model, not just a one-time report.
  • Put an approved commitment roadmap in place for Reserved Instances and Savings Plans.
  • Established a FinOps Center of Excellence (CoE), giving the client a reusable framework for future M&A.

Business impact

  • 27% reduction in overall cloud costs
  • ~$491K in annualized savings ($41K per month optimized)
  • ~$283K per year saved by re-architecting the Azure AVD pool, without a single outage
  • 2,160+ idle and oversized resources cleaned up, 95% of the identified scope
  • Zero downtime across all changes, delivered through approval-gated, maintenance-window execution

Next phase opportunities

  • A planned VDI-to-OCI migration expected to unlock ~$113K per year in additional savings
